Office Information

CPRS in Middleburg offers a wide variety of services including: Physical Therapy, Vestibular Therapy, Sports Rehabilitation, Pre and Post-Operative Therapy, Occupational Health, Oncology Rehab Program, Diabetes Management, and Wellness. We offer several wellness programs including: general membership, healthy lifestyles, personal training, and sports specific training.


Nicole, PT, MSPT Physical Therapist Clinic Manager
Nicole, PT, MSPT joined CPRS Physical Therapy in 2021. She is a graduate of Selinsgrove Area High School and earned her Physical Therapy degrees at Ithaca College in New York. Nicole has been in practice since 1999, working in outpatient therapy and home health in Rhode Island and Pennsylvania. Nicole currently resides in Lewisburg and in her spare time, Nicole enjoys books, fitness, food, traveling and spending time with family.

Breonna "Bre", PTA Physical Therapist Assistant
Breonna "Bre", PTA, has been with CPRS since 2021. Breonna is a graduate of Mifflinburg High School and received her Associate’s degree from Penn State University. Breonna lives in Mifflinburg and enjoys spending time with friends and family, playing field hockey, hunting and being outdoors.
